Before You Copy the Tool

This sample audit is built in Microsoft Forms.

To make your own copy and edit it for your organization, you'll need to be signed in with a Microsoft account that has access to Forms, such as Microsoft 365.  ​Once you open the link, choose the option to duplicate or copy the form into your own Forms account.  From there, you can customize the questions, branding, and settings for your facility.
