Before You Begin 

This audit tool is built in Microsoft Forms. To customize it for your facility, you'll need a Microsoft account with access to Forms, such as Microsoft 365. 

Open the Online Tool and select the option to duplicate or copy the form into your own Microsoft Forms account. You can then modify the questions, add your facility's branding, and adjust the settings to meet your needs. 

Tip: Test the form before using it for live observations, and confirm that responses are being collected in your organization's account.
