Norovirus is one of the leading causes of infectious gastroenteritis worldwide and is responsible for most acute gastroenteritis outbreaks. These outbreaks occur most often in settings where people are in close contact with one another—such as healthcare facilities, long-term care homes, schools, cruise ships, and food service settings.

The reason norovirus causes so many outbreaks is simple: it spreads extremely easily. Only a very small amount of virus is needed to cause infection, and once an outbreak begins, it can be difficult to control—especially in crowded or shared environments.

  • Shellfish, especially clams and oysters, when eaten raw or undercooked

  • Prepared foods such as salads, coleslaw, and egg-based dishes, particularly when contaminated during handling

  • Berries

  • Bakery products contaminated by infected food handlers

 

These foods and water sources are considered high risk because they are often consumed raw or handled after cooking, allowing the virus to survive and spread without being eliminated by heat.

    • The virus can easily transfer from contaminated surfaces to the hands and then into the mouth, often without a person realizing it. This is why hand hygiene, especially washing hands with soap and water, is so important in preventing the spread of norovirus. â€‹

  • Aerosolized through the air during vomiting

    • When someone vomits, tiny virus-containing particles can become aerosolized and remain suspended in the air for a short period of time. These particles can then be inhaled or settle on nearby surfaces, where they may later be transferred to the mouth by hands. â€‹

    • This has been demonstrated in real-world investigations. In a hospital outbreak, Alsved and colleagues collected air samples during episodes of norovirus illness and detected norovirus RNA in 21 of 86 air samples taken from rooms of 10 different patients. The amount of virus detected ranged from 5 to 215 genomic copies per cubic meter of air, with significantly higher concentrations found in samples collected within three hours of a vomiting event.​

    • A separate investigation by Marks and colleagues provides a striking example of how aerosolized norovirus can spread in community settings. Their study traced a restaurant outbreak back to a single episode of vomiting. After that event, 91 percent of diners seated at the same table became ill. At two nearby tables, 71 percent and 56 percent of diners developed symptoms. Infection rates dropped as distance from the vomiting event increased, supporting a dose-dependent pattern of exposure.

  • Sudden onset of vomiting, including explosive vomiting

  • Diarrhea, including explosive diarrhea

  • Nausea

  • Abdominal cramping

  • Low-grade fever

  • Headache, body aches, and fatigue

  • Signs of dehydration, such as dry mouth, decreased urination, dizziness, or confusion

  • Inability to keep fluids down due to ongoing vomiting

  • Severe abdominal pain

  • Do not shake or handle soiled laundry without care. This can spread virus into the air. Handle laundry gently and wash with detergent using the warmest appropriate water setting.

  • Do not return to work, school, or food preparation too soon. People should stay home for at least 48 hours after symptoms stop, even if they feel better. 

  • Do not share food, drinks, utensils, towels, or bathrooms with someone who is actively ill whenever possible.
